Continence management

Continence is the ability to control bowel or bladder function, but sometimes, for a range of reasons, people lose this control and need help to either regain continence or manage their continence issues.

Our nurses can:

  • help you better understand the cause of your problem
  • work with your doctor to manage your care
  • advise you on choosing the right management option
  • provide information on continence aids and purchase options
  • assess your eligibility for funding of continence aids
Dementia care

Dementia is the loss of cognitive ability in a person and can quite often be a frightening and confusing time for people. Bolton Clarke is here to help you manage these changes and keep you connected and happy.

Our nurses can:

  • assess and help you manage problems with memory loss and confusion
  • work with you on practical steps that allow you to live safely in your own home
  • help you manage your medicines and understand what they do and how they work
  • complete a comprehensive assessment with referrals to a range of relevant community services
Diabetes services

Diabetes is an imbalance in the body’s blood glucose system which results in high blood glucose levels.

Our nurses can:

  • help you manage your diabetes
  • provide education and support
  • help you monitor your blood glucose
  • help you manage your tablets
  • work with other health professionals like your doctor who are involved in your care
  • link you into community support programs which may help you i.e. the Diabetes Supply Scheme.
Palliative care

At Bolton Clarke we can provide palliative care if you have a terminal illness. In what can be a long and often difficult journey we’ll always try to preserve your quality of life and assist in managing end of life with dignity in the place you’ve chosen.

Our palliative care nurses can:

  • provide expert support anytime, day or night
  • help manage pain and other symptoms
  • provide emotional and spiritual support to you and your family, including overnight respite
  • arrange any necessary equipment
  • work in partnership with your family, doctor and community palliative care service
  • provide or arrange bereavement care including grief counselling
Wound care

Wounds are sores or breaks in your skin like ulcers. Sores are caused by pressure, tears on your skin or wounds that haven’t healed properly after an operation. Many wounds and skin problems heal more quickly and cause less problems if they are treated early by a nurse or doctor.

Our nurses can help you with your wound by:

  • looking at what caused your wound and helping you to address the problem
  • working with you on a plan for yourself or your carer to make sure that your wound is managed in the best way possible
  • working with your doctor and other health service providers to make sure that your treatment involves all the people looking after you
  • helping you to choose the most suitable and cost effective wound care products for your wound

It is never too early to get help to look after your skin.
