I give to Royal District Nursing Service Limited trading as Bolton Clarke (ABN 49 052 188 717), Level 1, 347 Burwood Highway, Forest Hill Vic 3131, for its general charitable purposes, free of all duties the sum of $ (insert your nominated amount) for which an authorised receipt from Bolton Clarke will be sufficient discharge for the executor(s) or trustee(s).
