Contact My Aged Care on 1800 200 422 to let them know you wish to transfer providers. They will reassign your Support at Home package to Bolton Clarke.
You'll also need to let your current provider know that you’ve decided to transfer to Bolton Clarke. They may require at least two weeks notice depending on your agreement.
Note: your unspent funds with your current provider will be transferred to Bolton Clarke.
Once your Support at Home package is reassigned, we’ll work with you (and your support person if you wish) to create your new personalised care and support plan.
