For people living with dementia it is important to receive the appropriate levels of care and support that meets their changing needs over time.
 
We offer specialist dementia care services that support the independence and wellbeing of people living with dementia at home, as well as in our residential aged care communities. With Bolton Clarke you will have access to best practice dementia care and the latest enabling technology.

At home dementia care

Support for individuals and their families

Bolton Clarke nurses and carers provide in-home dementia care, enabling clients to live independently. We work with clients and their families, to deliver tailored specialised care plans, including:

  • nursing, dementia and palliative care
  • medication, diabetes, pain and wound management
  • dignified personal and continence care

Our work also includes the development of the Information on Dementia talking book in English and Vietnamese

Residential aged care

For those who need additional support, our residential options provide around the clock care and peace of mind.

Our communities and Memory Support Centres provide home-like environments that are safe, supportive and encourage participation in social activities. Residents are supported to be as independent as possible, with thoughtfully designed environments and spaces to enjoy.

The services are staffed by expert nurses and dedicated carers who can always provide help, including emergency situations. We make sure the simple things that make life enjoyable are taken care of too.

Speak with a doctor if you have any concerns about your or a friend's or family member's memory, thinking or behaviour.

Contact My Aged Care on 1800 200 422 or myagedcare.gov.au for an Aged Care assessment.
This assessment will outline entitlements and level of care required based on individual needs.
