Palliative care

Personalised care and support at end of life

Bolton Clarke’s palliative care nursing services provide choice and support for people managing the symptoms of life-limiting illness.

We deliver compassionate care with a focus on dignity, choice and comfort – whether it's at home surrounded by family and friends or in a residential aged care community.

Collaborative care

Bolton Clarke nurses and carers work with our clients and their families to deliver tailored support including:

  • palliative, nursing and dementia care
  • medication, diabetes, pain and wound management
  • dignified personal and continence care

As part of a holistic care plan, we also work with GPs and Specialist Palliative Care Services to develop individual care plans.

Specialist Palliative Care Services offer support from palliative care doctors and nurses and additional therapies.
