For Bolton Clarke client Ron, receiving help with medication management through a partnership between Bolton Clarke and Eastern Melbourne Primary Health Network was life-changing.
The TEAMM (Timely Enhanced Access to Medication Management) project sought to improve medication safety for older clients by providing access to a clinical pharmacist who visited them in their home.
“I used to love going to the footy, but I haven’t been able to go for the last year because of illness,” Ron says.
“I was taking about 20 tablets in the morning. I had one of the nurses around and was talking to her about going to sleep and dropping cups of coffee.
“The TEAMM Pharmacist changed my life. I’m 100 per cent better off than what I was.”
Medication errors occur in up to 40 per cent of older people, and about 13 per cent experience adverse medication event resulting in hospitalisation.
Results from the TEAMM project are now being evaluated.
